Increased reports of "can't teleport more than 6 times per minute" when trying to TP

For the last few days Firestorm support has been seeing reports from users who are getting the error "can't teleport more than 6 times per minute" when trying to TP. The incidence seems higher today.
I'm filing this to create a place for users to add their cases.
Users, please provide as much of the following info as possible:
• Were you were trying to TP into a region/parcel running an Experience?
• What viewer version are you on?
• What region were you trying to reach?
• What time did this happen?

All of the points of information Anastasia Horngold mentioned are necessary for this investigation. In general, the "can't teleport more than 6 times per minute" message can trigger when the viewer fails to TP to a destination region. The throttle (and corresponding error message) gets triggered by automatic retries when the destination region is unavailable.
